Newborns may have transient breast enlargement from maternal hormones, adolescents may develop temporary pubertal gynecomastia, and older men are more likely to have age-related hormonal decline, medication exposure, or chronic disease. These age-related patterns influence whether observation or broader testing is appropriate.

Severity and duration also matter. Mild, recent, and symmetric enlargement is more likely to be benign and transient, while rapidly enlarging, painful, or unilateral masses deserve closer evaluation. Long-standing gynecomastia may become fibrotic and less tender, but chronicity does not exclude a treatable cause if the underlying hormone imbalance persists.

Associated medical conditions can broaden the workup. Obesity can obscure the distinction between fatty tissue and glandular enlargement. Liver disease, renal disease, thyroid disease, infertility, sexual dysfunction, and testicular abnormalities all raise the likelihood of a systemic or endocrine cause. Drug history is equally important because medication-induced gynecomastia is common and often reversible when the triggering agent is removed.
